729 Endoluminal Vacuum Therapy in the Management of Boerhaave Syndrome: A Case Series

Abstract Background Boerhaave syndrome is a rare condition associated with poor prognosis: high morbidity and mortality. Prompt intervention greatly improves outcomes, surgery being the mainstay of management. Recent advances in therapeutic endoscopy have led to increasing interest endoluminal vacuum therapy (EVT). EVT minimally invasive technique, allowing wound debridement drainage, granulation tissue formation. has been excellent clinical including lower mortality rates when compared stenting. adopted into practice across Europe, however, only two cases reported UK. We report three syndrome, successfully managed EVT, using EsoSPONGE ® (B.Braun, Medical Ltd, Sheffield, UK). Method involves placement polyurethane sponge cavity. The cavity initially assessed endoscopically before an overtube introduced. pushed through overtube, removed. Sponge position confirmed adjusted if necessary. connected via trans-nasal drain continuous negative pressure suction changed every 3–5 days. Results Having deemed surgically unfit, all patients were referred for EsoSPONGE. All made recovery discharged home. Conclusions effective management strategy unfit patients. use aided drainage septic focus closure defect, leading complete recovery. Our findings support existing evidence that promising solution syndrome.
